About the Position
Charter House is Mayo Clinic’s retirement community located in the heart of downtown Rochester, Minnesota. In this role, nurses play a vital role in delivering exceptional care and creating meaningful connections with residents and families. The position involves caring for older adults in assisted living and short‑term rehabilitation centers, providing a safe, caring, and engaging community that puts residents’ needs first.
Responsibilities- Support high‑quality resident care by completing timely, accurate clinical assessments and applying the nursing process to develop, implement, and update individualized care plans.
- Demonstrate expertise in geriatric care, managing chronic conditions, frailty, functional decline, and cognitive impairment while integrating geriatric-informed and dementia‑capable practices.
- Collaborate with the interdisciplinary team to coordinate services, ensuring care plans, documentation, and assessments meet organizational standards, evidence‑based practice, and regulatory requirements.
- Oversee wound care, coordinating monitoring and interventions, and serve as a clinical resource to staff for wound prevention and management, maintaining wound care certification.
- Provide clinical leadership by serving as a role model, mentor, and resource for nursing staff, supporting care delivery and unit operations in the absence of the Nurse Manager.
- Use clinical and non‑clinical software systems to maintain accurate records and contribute to a culture of quality, safety, and regulatory readiness.
This position is not eligible for visa sponsorship with the exception of the TN visa classification;
Mayo Clinic does not participate in the F‑1 STEM OPT extension program.
Required:
- Associate degree in nursing and two years of RN experience in a hospital, long‑term care facility, or other health care facility.
- Active RN license on the Minnesota Board of Nursing.
- Basic Life Support for Health Care Providers certification from American Heart Association or American Red Cross.
Preferred:
- Bachelor's degree in nursing (BSN).
- Two years of nursing experience in long‑term care, including dementia care or assisted living.
- One year of acute care nursing experience.
- Wound care certification.
- Experience in MDS coordination and MDS certification.
- Demonstrated leadership and effective communication skills.
- Excellent critical thinking, problem‑solving, and organizational skills with the ability to prioritize work.
- Ability to self‑direct and work independently.
- Adaptability to unpredictable situations and stress.
- Flexibility to work days, evenings, nights, holidays, weekends, and on‑call shifts as needed.
- Computer literacy and strong knowledge of software applications.
