ResponsibilitiesFamily Medicine is located within the Baldwin Building, NW Clinic, NE Clinic, SE Clinic, and Kasson Clinic. The exact current practice location within Rochester will be determined based on practice needs at the time of hire.
The NP or PA will work in a care team setting in collaboration with other team members including physicians and other allied health staff.
The NP or PA will be involved in population management strategies to improve chronic illness outcomes.
The provision of care includes but is not limited to comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments, non-pharmacologic therapies, and counseling patients.
Qualifications- Nurse Practitioner: Masters prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current Minnesota RN and APRN license and certification as an FNP. DNP is preferred.
- Physician Assistant: Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the Minnesota State Board of Medical Practice. DMSc is preferred.
BLS certification and DEA registration required.
Additional Qualifications- NP or PA provider experience preferred.
- Experience in primary care preferred.
- Previous experience working with pediatric as well as adult patients preferred.
- The candidate should be comfortable with autonomy, independent decision making, and possess excellent communication skills.
- The preferred candidate will be a team player, who is comfortable seeing patients of all ages. He/she will function as a member of a care team, seeing patients for chronic disease management, preventive care, health promotion and acute care.
