What You'll Do
- Provide routine and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing settings.
- Perform comprehensive nursing assessments, symptom management, medication education, and comfort-focused interventions.
- Support patients and families through compassionate communication and ongoing education.
- Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
- Complete accurate and timely EMR documentation while maintaining high clinical standards.
What You'll Bring
- Active Minnesota Registered Nurse (RN) license.
- Hospice experience preferred, or strong experience in oncology, palliative care, home health, ICU, or end-of-life care .
- Excellent clinical assessment and communication skills.
- Ability to work independently in a field-based role.
- Reliable transportation and a valid driver's license.
