As a Mobile Case Manager (MCM), you're the bridge between shelter and community, the consistent connection that helps youth stay stable and supported long after they've exited our doors. You're out in the community, in the spaces where youth naturally gather, making sure no one falls through the cracks during a critical transition.
This role sits at the heart of 180 Degrees' continuity-of-care model. You provide prevention-focused, community-based support and structured aftercare that keeps youth connected to stability as they move from shelter back into daily life. You collaborate closely with shelter staff and fellow Mobile Case Managers across the organization, ensuring every youth experiences a consistent, unified journey no matter where they are or what program they're in.
You'll operate with independence in the field but you're part of the Shelter team. This role is grounded in strong supervision, teamwork, and constant connection back to shelter operations.
