A Social Services Coordinator coordinates care planning, psychosocial assessments, admissions, and discharge processes by collaborating with interdisciplinary teams, residents, and families to ensure appropriate services, referrals, and support are in place.
Maintains knowledge of community resources, regulatory requirements, and third‑party payer programs, while facilitating care conferences, developing resident care plans, and providing education on resident rights, psychosocial needs, and related topics.
Must be able to work in collaboration with others, be knowledgeable about Medicare and other third‑party payors, possess good communication and human‑relation skills, and demonstrate strong problem‑solving ability while adapting to unpredictable situations.
Qualifications- Bachelor's degree from an accredited college or university in Social Services or Social Work.
- Prior experience working in a group setting in healthcare, preferably post‑acute rehabilitation and/or long‑term care.
- Preferred license by the Minnesota Board of Social Work at a minimum an LSW (Licensed Social Worker).
- Candidates who have recently taken the Minnesota Board of Social Work license test or have a scheduled test date may apply.
- One year of work experience in Long Term Care or Home Health is preferred.

Compensation Detail$25.42 - $36.85 / hour

Hours are generally 8:00am‑4:30pm.
