Cornerstone VNA is seeking a compassionate and detail-oriented Intake Nurse (LPN) to join our Intaketeam. The Intake Nurse plays a vital role in coordinating and streamlining the flow of medical records, referrals, and clinical communications between internal clinical teams and external providers. This position supports the timely and accurate processing of referrals, physician orders, and triage tasks (within licensure), ensuring patients receive the right care at the right time.
This position offers flexibility and may be in-person, hybrid, or remote, depending on agency needs and the candidate's qualifications and work preferences.

This job does have a Saturday Shift Requirement each week

What You'll Do

Coordinate home health services by collaborating with clinical management, coordinators, provider offices, hospitals, and other referral sources.

Receive and process physician verbal and written orders, documenting per process and communicating with clinical staff or managers.

Receive and process SBARs (Situation-Background-Assessment-Recommendation), ensuring timely communication with staff or managers.

Coordinate referrals for all programs including home care, perinatal, IV therapy, rehabilitation, social services, hospice, community palliative care, and telehealth.

Triage calls related to referrals, physician orders, and SBARs.

Participate in CVNA's emergency preparedness processes and respond to emergency building alerts.

Perform additional responsibilities as assigned in an accurate and timely manner.

What You Bring

Current NH and Maine LPN license (or any multi-state license)

2+ years of clinical nursing experience preferred (home care experience a plus)

Ability to work independently while being part of a collaborative care team
