LVN​/LPN – LTC​/SNF

Job Summary:

Providing skilled nursing care as an LVN/LPN within a long-term care or skilled nursing facility setting. This role involves delivering quality patient care while adhering to facility protocols and regulatory standards.

Responsibilities:

• Administer medications and treatments accurately according to prescribed plans.
• Monitor, document, and report patient conditions and responses to care.
• Collaborate with healthcare team members to develop and implement care plans.
• Ensure patient safety by maintaining proper infection control and safety procedures.
• Provide compassionate support to residents, maintaining their dignity and comfort.
• Maintain detailed, timely documentation for each patient encounter.

Qualifications:

• Valid LVN or LPN license in the state.
• Proven experience in long-term care or skilled nursing facilities preferred.
• Ability to follow detailed protocols and write comprehensive documentation.
• Excellent communication and interpersonal skills.
• Demonstrated crisis management and problem-solving abilities.
• Must comply with COVID-19 vaccination or exemption requirements.
