LVN​/LPN – LTC​/SNF

Job Summary:

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure patient comfort, safety, and compliance with all regulatory standards.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Monitor resident health status and promptly report any changes.
• Document patient observations and care activities thoroughly.
• Follow infection control protocols and maintain a safe environment.
• Collaborate with interdisciplinary team to develop and update care plans.
• Ensure compliance with facility policies and regulatory requirements.

Qualifications:

• Valid LVN or LPN license in the state of employment.
• Minimum of one year of experience in LTC or SNF settings preferred.
• Ability to provide compassionate, patient-centered care.
• Strong communication and documentation skills.
• Must have completed the COVID-19 vaccination series or possess valid exemption documentation.
• Ability to work weekends and flexible shifts as required.
