Waiver Services Coordinator

Salary

$20.00 - $23.00 per hour commensurate with experience

Employment

Full-time 30-hour work week. Mon-Fri between the hours of 8am – 4pm

Requirements
  • Associate’s degree in business or health related field and two years experience. In some cases, an equivalent combination of education and experience may be acceptable.

  • Must have computer literacy skills and a demonstrated ability to adapt to new computer software.

Summary/Objective

To coordinate consumer services from intake/admission through discharge for all OPWDD Waiver services. To support department or program staff as requested in meeting all agency and regulatory requirements.

Key Job Tasks/Essential Functions
  • Provides follow up to all calls for agency information and referrals for OPWDD Waiver Services.
  • Creates intake packet to include all required documents per program; communicates with families/care coordinators and other providers as needed to gather information as requested by program staff.
  • Works with agency admission committee members to review any new candidates for services. Works with program staff, consumers, families and providers by phone or in person, scheduling on site visits and meetings as necessary.
  • Processes admission to new consumers in accordance with department or program guidelines and agency practices.
  • Reviews, updates, processes, and maintains all records in accordance with the standards of regulatory agencies governing the operation of CP.
  • Maintains computer records, demographic and billing information in accordance with department or program guidelines and agency practices.
  • Collects billing, follows through on billing problems, and ensures the accuracy of billing and income information.
  • Transition apt manager- assists with collecting and processing of rent, maintenance and lease violations, etc.
  • Achieves program goals by performing other responsibilities as required.
Key Relationships
  • Directly supervised by Director of IFSS and Director of Waiver Services.
  • Interacts regularly with consumers, families/caregivers, department or program staff, finance/accounting staff, insurers, and external service providers to ensure a smooth transition into programs.
