Rochester Regional Health Home Care is seeking a Team Case Assistant to coordinate patient services under the supervision of the team manager. The role includes prioritizing daily schedules, coordinating additional services, and ensuring timely referrals.
You will manage calls, assist field staff, monitor email, maintain EMR, support new staff orientation, attend meetings, and perform general office duties in a 24-hour/week schedule (Friday–Sunday).
