Rochester Regional Health is seeking an LPN Intake Coordinator for Home Care at 330 Monroe Avenue, Rochester, NY 14607. This 40-hour/week role works 8:00am-4:30pm or 8:30am-5:00pm, Monday through Friday, coordinating referrals and data entry for home visits.
The ideal candidate is an NYS-licensed LPN with nursing AS, office experience, and EMR/Microsoft Office proficiency to ensure smooth transitions to home care and excellent patient service.
