Position Overview The Primary Care Physician will provide comprehensive onsite primary care services to eligible
Veterans within the VA Finger Lakes Healthcare System (VAFLHCS). Care must meet or exceednationally recognized standards established by the American Board of Family Medicine (ABFM) or the American Board of Internal Medicine (ABIM).

- Rochester VA Outpatient Clinic – 260 Calkins Road, Rochester, NY 14623
- Wellsville VA Outpatient Clinic – 3458 Riverside Drive, Wellsville, NY 14895
- Wellsboro VA Outpatient Clinic – 1835 Shumway Hill Road, Wellsboro, PA 16901
- Elmira VA Outpatient Clinic – 1316 College Avenue, Elmira, NY 14901
Key Responsibilities – Clinical Duties
- Provide comprehensive primary care services to adult Veteran patients
- Diagnose, treat, and manage acute and chronic medical conditions
- Conduct preventive care, health screenings, and wellness visits
- Manage chronic diseases such as diabetes, hypertension, COPD, and heart disease
- Prescribe medications and order diagnostic testing as clinically indicated
- Coordinate care with specialty services and ancillary staff
- Ensure timely, accurate documentation in the VA Electronic Health Record (EHR)
- Provide patient education and continuity of care
- Participate in quality assurance, peer review, and performance improvement activities
- Comply with VA policies, clinical practice guidelines, and performance metrics
- Attend required staff meetings and complete mandatory VA training
- Maintain productivity and documentation standards in accordance with VHA directives
- MD or DO from an accredited medical school
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active, unrestricted medical license in any U.S. state or territory
- Active DEA registration
- Ability to complete VA credentialing and privileging requirements
- Compliance with VA immunization and background requirements
