Location: City of Rochester
Position Overview
We are seeking a Board Certified or Board Eligible Primary Care Physician to provide comprehensive outpatient care to Veterans within a large, integrated federal healthcare system in the Finger Lakes region. This is an onsite role focused on high-quality, patient-centered care with a predictable schedule and strong interdisciplinary support.
You will serve as the lead clinician for a Patient Aligned Care Team (PACT) and manage a balanced panel of patients in a setting designed to prioritize continuity, quality, and work-life balance.
Key Responsibilities Clinical Care- Provide primary care services to an average of 12–13 patients per day
, including acute, new, and routine visits - Perform comprehensive histories and physical examinations
- Diagnose and manage acute and chronic medical conditions
- Develop individualized treatment plans
- Complete all clinical documentation using CPRS
- Close all patient encounters by the end of each shift
- Utilize VistA Imaging for clinical documentation and order entry
- Direct ongoing outpatient care
- Conduct preventive care, health education, and disease prevention activities
- MD or DO from an accredited medical school
- Board Certified or Board Eligible in:
- Family Medicine (ABFM), or
- Internal Medicine (ABIM)
- Current, full, and unrestricted medical license in any U.S. state or territory
- Active BLS and ACLS certification (or equivalent)
- Active DEA registration
- National Provider Identifier (
NPI
)
- Fluency in spoken and written English
- Proficiency with CPRS and VistA Imaging
- Ability to manage diverse patient populations and complex medical conditions
- Strong diagnostic and communication skills, including timely follow-up of test results
- Monday–Friday, 7:30 a.m. – 4:00 p.m.
- 40 hours per week
- No federal holidays (11 annually)
