SUMMARY: The position will support the RRH Quality and Safety strategic pillar to provide the highest quality care, free from preventable harm with best in‑class service at the lowest cost. This role will have responsibility for the clinical patient safety operations of Rochester Regional Health. They will utilize leadership and operational expertise plus knowledge of quality and patient safety science, theories of innovation to recommend and operationalize RRH quality and safety goals, and organize patient safety resources to mitigate clinical risks and ensure the highest possible level of safety, quality, performance and innovation.

KEY RESPONSIBILITIES:

PEOPLE LEADERSHIP
  • Effectively recruits and retains talent for continuous support of aligned business areas.

  • Monitors and recognizes performance of people and supports people in professional development. Supports workplace well‑being and workplace safety.
PROGRAM DEVELOPMENT AND IMPLEMENTATION
  • Design, implement, and evaluate patient safety programs, policies and procedures across all facilities within the health system. (e.g. CUSP, Safety Culture Assessment).
  • Integrate patient safety principles into clinical workflows, processes, and systems.
INCIDENT RESPONSE & INVESTIGATION
  • Oversee the operational use of the event reporting system.
  • Oversee the investigation and analysis of patient safety incidents, adverse events, and clinical risks, utilizing root cause analysis methodologies to identify contributing factors and opportunities for improvement.
  • Partner with senior leadership to develop and implement proactive strategies to prevent future occurrences and enhance patient safety outcomes.
REGULATORY COMPLIANCE AND ACCREDITATION
  • Ensure internal business processes are compliance with all relevant regulatory requirements, accreditation standards, and guidelines related to patient safety.
  • Serves as a subject matter expert on patient safety issues during regulatory surveys, audits, and accreditation visits.
EDUCATION and TRAINING
  • In partnership with senior leadership, develop educational programs, training sessions, and resources to enhance awareness of patient safety principles, risk mitigation strategies, and incident reporting processes among staff, clinicians, and leadership.
  • Delivers operationally on the educational programs and training sessions.
  • Foster a culture of learning and continuous improvement by promoting interdisciplinary collaboration, knowledge sharing, and evidence‑based practices.
DATA ANALYTICS AND PERFORMANCE MONITORING
  • Utilize data analytics and performance metrics to monitor patient safety outcomes, identify trends, and benchmark performance against industry standards and internal targets.
  • Provide regular reports and presentations to senior leadership and governing bodies on patient safety initiatives, risk management activities, and quality improvement efforts.
MINIMAL QUALIFICATIONS
  • Effective 01/01/2028, Master's Degree required.
  • Bachelor’s degree required.
  • 3 years of experience in healthcare quality and patient safety (or other relevant experience) required.
PREFERRED QUALIFICATIONS
  • Strong knowledge of safety science, high reliability, improvement methodologies, regulations, and accreditation standards (e.g., NYS, DNV, CMS).
  • Demonstrated ability to apply implementation science methodologies (Safety culture, human factors, design thinking, root cause analysis, qualitative analytics and implementing risk mitigation strategies).
  • Excellent communication, collaboration, and interpersonal skills with the ability to engage and influence stakeholders at all levels of the organization.
  • Certification in patient safety (e.g., CPPS), risk management (e.g., CPHRM), or quality improvement (e.g., CPHQ) preferred.
  • Highly proficient in analytical and data management and interpretation skills.
  • Excellent leadership, communication, and team‑building skills.
  • Ability to work collaboratively with all levels of staff and adapt to changing healthcare environments.
  • High level of proficiency with Microsoft Excel, Teams and PowerPoint.

PHYSICAL REQUIREMENTS: Sedentary work – Exerting up to 10 pounds of force occasionally. Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
