Hospital Aide - Hospice Care (Per Diem)

DEPARTMENT: Inpatient Hospice Care

LOCATION: Hildebrandt Hospice Care Center, 2652 Ridgeway Avenue, Rochester, NY 14623

HOURS PER WEEK: Per Diem (as needed)

SCHEDULE: Per diem. Shifts run 7:00am-3:30p (days), 3:30p-11pm (evenings) and 11:00pm-7:00am (nights)

Summary

Under the supervision of the Assigned Management, this position provides the quality care necessary to restore or maintain the patient’s physical and emotional well-being. The position assists with the physical and personal care of clients and their activities of daily living, including assistance with meals, maintenance of safety and environmental upkeep. The aide assists in maintaining a supportive and empathetic environment for patients and families.

Care

Hospice care is specialized end‑of‑life care that gives patients and families the chance to spend time together with comfort and dignity, receiving services focused on sustaining quality of life.

Responsibilities Level I
  • Follows Hospice Plan of Care for each individual patient as directed by the RN.
  • Observes patients’ conditions and reports any and all changes in patient condition to the RN.
  • Turns and repositions bedridden patients alone or with assistance.
  • Feeds patients who are unable to feed themselves. Serves and collects meal trays.
  • Measures and records food and liquid intake/output and vital signs – reports any changes to professional staff.
  • Provides patients assistance with walking, exercising and moving in and out of bed, if needed.
  • Provides assistance with personal care – bathing and grooming.
  • Answers patient call lights.
  • Gives regular attention to the patient’s comfort and communicates perceived needs to the nursing staff.
  • Maintains a neat and clean patient area.
  • Changes linens as needed.
  • Completes light housekeeping as necessary.
  • Stocks/stores medical supplies and linens.
  • Offers support and comfort to families at the time of patient’s death.
  • Provides post‑mortem care.
  • Assists RN and other disciplines as it relates to health and non‑health related tasks.
  • Consistently demonstrates high standards of integrity by supporting the Lifetime Healthcare Companies’ mission and values and adhering to the Corporate Code of Conduct.
  • Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures.
  • Regular and reliable attendance is expected and required.
  • Performs other functions as assigned by management.
Level II

In addition to Level I responsibilities:

  • Provides mentoring/support to other Home Health Aide(s) as requested.
  • Provides hands‑on experience to new Home Health Aide(s).
  • Works with patients and their respective caregivers who need additional support.
Required Qualifications Level I
  • High School Diploma or equivalent.
  • Completion of a Hospice Orientation within 3 days of hire before working as a Hospice Aide.
  • At least one year of nursing assistant or Home Health Aide experience.
  • HHA Certified or Ability to meet one of the following eligibility requirements for Competence:
    • Currently certified CNA.
    • Documented home health aide or nurse aide training and competency evaluation from an out‑of‑state training program.
    • Home health aide with documented home health aide training and competency evaluation who has not been employed as a home health aide for 24 consecutive months.
  • Those qualifying with a CNA, an out‑of‑state training program, or as an HHA without 24 consecutive months of employment must complete Lifetime’s competency program and earn active New York State HHA certification within 4 days from hire.
  • An interest and sensitivity in caring for the terminally ill.
  • Ability to read, writes, understands and carries out directions/instructions and maintains simple records.
  • A pre‑employment drug test and physical examination required and an annual health assessment (as required by NYS Health Department) are musts.
  • Must respect patient confidentiality.
  • Accomplishes duties of the job within allotted time.
  • Work is thorough and neat.
In Addition To Level I Qualifications
  • Must have two years of Hospice Aide experience.
  • Participates in the training of new agency home health aides (CHHA) during their initial training/certification.
