General Description

Under general supervision, provides intake and case management services. Maintains confidentiality and ethical standards as identified by NASW, HIPAA, or other relevant governing bodies. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma-informed care environment.

Essential Duties and Responsibilities
  • Maintains a caseload of clients from the URMC Respite program of up to 15 clients and maintains their case files by Agency, departmental, regulator, and funder guidelines. In addition, also provides facilitation of CCFCS’s emergency shelter telehealth program. This position also provides coverage and support as needed to other programs as needed by the program manager.
  • Completes relevant screenings and assessments including but not limited to psychosocial, mental health, financial status, and employment status.
  • Creates, implements, and monitors comprehensive, person-centered, trauma-informed, and solution-focused plans and goals. Works with clients on harm reduction and other related goals.
  • Assists in obtaining support for clients to include but not limited to home care, medical, dental, legal, substance use treatment, mental health, Department of Human Services, clothing, and other entitlement needs.
  • Assist the client in understanding and meeting the expectations of service providers, courts, and employers.
  • Inform clients of opportunities for social support such as recreation, faith-based, or self-help involvement. Make available tickets, transportation, and financial assistance where appropriate.
  • May deliver instructions on various topics that support the client’s ability to live independently.
  • Provides ongoing evaluation, supervision, and development of program activities to assure quality and effectiveness of services provided.
  • Implements and maintains procedures related to program operations.
  • Submits required reports and statistics promptly to the program manager as requested.
  • Collaborates effectively with program partners to heighten the efficiency of joint efforts across multiple organizations.
  • Communicate with appropriate program partners, agency staff, community, and criminal justice agencies to ensure ongoing and effective referrals into programs.
  • Serves as Case Coordinator for clients for program care. Works with the Nurse, Peer, and other stakeholders to ensure the client's needs are being met. Coordinates and participates in regular case conferences.
  • Monitors and ensures achievement of program milestones and client outcomes.
  • Oversees client recordkeeping in compliance with appropriate regulatory agencies.
  • Creates and implements life skills training and education opportunities for tenants.
  • Collaborates with referring agencies, funders, and all related internal staff to effectively support individual client needs.
  • Maintains regular contact to coordinate client benefits and address all other treatment needs.
  • Reports to service providers about significant individual behavior or events that will require an immediate response.
