Provide social services to patients in the home, and clinical direction to fellow Social Workers and other clinical staff.
Essential Functions
- Carryacaseload and perform job duties as described for an MSW.
- Assist fellow MSWs byprovidingguidance on patient issues,as well as community and financial resources.
- Ensure appropriate cross-referrals are being made, withineachdesignated region.
- Assistin staff orientation as needed.
- May conduct field supervision of Social Work Assistants.
- May review and co-sign orders obtained by Social Work Assistants.
- Mayassistin arranging guest speakers and in-services for Social Work Dept.
- Assist with in-house presentations as needed.
- Develop andmaintainstrong community outreach efforts on behalf of HCR Home Care.
- Other duties as assigned.

Education Requirements
- Master’s degree fromaschoolaccredited by the Council of Social Work Education.
Qualifications and Requirements
- Licensed Master of Social Work (LMSW)/Licensed Clinical Social Worker (LCSW) in NYS.
The Medical Social Workeris primarily in an office setting and may be exposed to outdoor conditions.
The working conditions are classified as light work:
- Light work - Exerting up to 20 pounds of force occasionally, and/or a negligible amount of force frequentlyor constantly to lift, carry, push, pull or otherwise move objects, including the human body. Light work involves sitting most of the time.
