Department: Social Services
Facility Location: 1555 Long Pond Road, Rochester, NY 14626
Shift: 8-Hour Evening Shift
Guaranteed Hours: 40 hours/week
Contract Length: 6 Weeks
Position Overview:
We are seeking a dedicated Social Worker for a 6-week contract to support our Social Services team. This role requires strong experience in hospital settings, particularly with discharge planning. Familiarity with the Rochester area and proficiency in EPIC are highly preferred. Candidates with either a Bachelor's or Master's degree in Social Work are welcome to apply. Local candidates may be considered.
Key Responsibilities:
- Conduct comprehensive psychosocial assessments and high-risk evaluations through patient/family interviews and chart reviews.
- Collaborate with the interdisciplinary care team to develop and document individualized care and discharge plans in alignment with clinical and regulatory standards.
- Actively participate in daily rounds, care coordination, and patient transition planning across the care continuum.
- Provide guidance on placement options and interpret federal, state, and local placement guidelines.
- Serve as a resource for emotional, social, and legal concerns, including guardianship, abuse/neglect, and advance directives.
- Advocate for patients/families by coordinating with Financial Case Managers, insurance providers, and healthcare partners to secure necessary coverage.
- Deliver age-appropriate interventions, including crisis intervention, counseling, education, and community referrals.
- Ensure timely execution of discharge plans and communicate with all relevant parties within 24 hours of discharge authorization.
- Bachelor's or Master's degree in Social Work (required)
- Hospital experience preferred
- Experience with EPIC EMR system
- Knowledge of discharge planning processes
- Strong understanding of local community and care resources (Rochester area preferred)
