Job SummaryUnder general direction of the Highland Women’s Health (HWH) Nurse Manager and Division Director, the OB RN Care Manager’s central role integrates and coordinates access and utilization management, proactive patient management, care facilitation and treatment planning functions for the obstetric patient population throughout the HWH practice. The RN Care Manager facilitates clinical aspects of a patient-centered medical home, working with the interdisciplinary medical team in the provision of patient care.
May also participate in supervision of other clinical staff members.
- Integrate and coordinate access and utilization management for obstetric patients.
- Provide proactive patient management and care facilitation.
- Assist in treatment planning functions.
- Facilitate clinical aspects of a patient-centered medical home.
- Collaborate with interdisciplinary medical team to provide patient care.
- Participate in supervision of other clinical staff members (as required).
- Licensed RN in NYS (BSN preferred).
- Three to five years of professional nursing experience.
- Outpatient OBGYN nursing experience preferred.
- Progressive clinical experience; community health, care management, and/or disease management preferred.
- Experience working as a member of a multi-disciplinary team required.
- Supervisory experience desirable.
- Experience in data analysis preferred.
- Registered nursing license.
- Fetal monitoring certification (NCC or PQF) current or to be obtained within 6 months of hire.
- Current CPR certification required (AHA BLS for health care providers or ARC CPR for professional health care providers).
- Successful completion of medication proficiency examination.
- Valid New York State driver’s license and ability to travel.
Salary range: $77,220 – $93,600 annually.
