Director of Admissions

A well-established skilled nursing and rehabilitation facility in New York is seeking an experienced Director of Admissions to oversee the full admissions process and support census stability. This role is ideal for someone who understands post-acute placement, payer requirements, and how to manage referrals efficiently in a fast-paced long-term care environment.

Responsibilities:

• Oversee the admissions process from referral through admission

• Review clinical documentation to ensure appropriate placement

• Coordinate with hospitals, case managers, discharge planners, and internal teams

• Manage insurance verification, authorizations, and payer requirements

• Track referrals, conversions, and census data

• Work closely with nursing, social services, and administration to ensure smooth transitions

• Serve as a point of contact for families throughout the admissions process

• Maintain compliance with all admissions-related regulations and policies

Qualifications:

• Prior admissions experience in a skilled nursing facility required

• Strong understanding of Medicare, Medicaid, and managed care admissions

• Knowledge of hospital discharge planning and post-acute care placement

• Excellent communication, organizational, and follow-up skills

• Ability to manage multiple referrals and priorities independently
