Location: City of Rochester
House of Mercy offers a 24/7 emergency shelter and wraparound services. The Street Outreach Coordinator will work with the HoM Street Outreach team to provide individualized living plans and outreach to underserved areas, collaborating across agencies to connect participants with housing, health, and support resources.
The role requires a Bachelor’s degree, 2–4 years of outreach experience, and a NY driver license, with evening and weekend hours as needed for field work.
