*** Responsibilities:
** GENERAL

PURPOSE:

In an outpatient setting, remains accountable for provision of direct care to assigned patients. Contributes to unit activities and supports enhancement of the care delivery system. Works as a team member and assists other staff members with implementing patient care. Assumes responsibility of care for assigned patients under the direction of the co-assigned RN or MD. Participates in the plan of care, implements appropriate nursing interventions, and contributes to patient/family education through collection of data, interdisciplinary team collaboration, documentation and excellent communication.

Full time position, Monday-Friday, 8am-4:30pm, no weekends or holidays. Ambulatory Neurology Triage LPN provides clinical support in the outpatient neurology setting by managing telephone and patient portal communications, performing patient blood draws, and assisting with in-clinic procedures. This role gathers and relays patient information to the care team, documents accurately in the electronic medical record, and works collaboratively with RNs and providers to support efficient clinic workflows and timely patient care.

6 months LPN experience required.
