This is a full-time, on-site role located in the Rochester, New York Metropolitan Area. The Nurse Practitioner will provide patient-centered care, including conducting thorough health assessments, diagnosing and treating acute and chronic illnesses, and developing individualized treatment plans. Responsibilities include prescribing medications, promoting health and wellness, and collaborating with multidisciplinary teams to ensure comprehensive care. The role also involves educating patients and families, advocating for healthcare access, and participating in community outreach initiatives.
Qualifications- Strong clinical skills in patient assessment, diagnosis, and treatment of acute and chronic conditions
- Experience with patient education, health promotion, and wellness management
- Knowledge of healthcare access programs, prescription assistance, and working with underserved communities
- Excellent communication, collaboration, and interpersonal skills
- Flexibility, adaptability, and a commitment to providing equitable care
- Master’s degree in Nursing and current Nurse Practitioner licensure in New York State
- Minimum of 2 years' post-graduate experience as a Nurse Practitioner, with a focus on primary care or community health preferred
- Current certification in Basic Life Support (BLS) and prescriptive authority
