Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.
FT: 40 hours, Days/Evenings/Weekends. The Acute Stroke Navigator (RN) role involves facilitating patient care during acute stroke alerts and collaborating with the interdisciplinary team including Emergency Medicine and Nursing, Stroke Neurology, and Neurosurgery to ensure efficient, guideline-driven treatment. Responsibilities include supporting patients through acute stroke evaluation and treatment and post-treatment monitoring, contributing to data collection and quality improvement initiatives aimed at optimizing stroke care outcomes, and facilitating transitions in care through the hospital and s is an opportunity for an experienced nurse to play a vital role in acute stroke management and patient outcomes.
Must possess strong assessment and communication skills, be proactive and independent in decision-making and have good organizational skills.
