Full-time (Monday – Friday, daytime hours) Nurse Practitioner position in a busy Family Medicine practice in Batavia, NY utilizing the Patient Centered Medical Home model. Fast‑paced, semi‑rural, community‑based practice with opportunity to work with a variety of patients, including Spanish‑speaking patients (Language Line interpreters available). Providers care for patients across all ages and medical issues.

• Evaluation and management of acute and chronic illnesses
• Ordering, interpreting, and communicating diagnostic tests and results
• Performing complete physical examinations and wellness visits
• Hospital/ED follow‑ups and transitional care management
• Collaborating with network‑based physicians and the medical team regarding patient care
• Facilitating appropriate referrals to specialists or community‑based agencies
• Undertaking office‑based procedures when appropriate
• Providing patient education and counseling to maintain or restore physical and emotional health and function
• Delivering educational support for learners consistent with the goals of the University of Rochester teaching institution
• Establishing a team‑supported patient panel

• Master's degree as a Nurse Practitioner
• Active New York State Registered Nurse license in good standing
• Active New York State Nurse Practitioner licensure in good standing
• Current national certification as a Nurse Practitioner
• NYS NP certification and licensure
• DEA registration applied for prior to hire/start date
• Greater than 2 years of active practice (NP preferred)
• Excellent communication and collaboration skills
• Clinical competence, superior verbal and written communication, interpersonal effectiveness, customer relations, computer competence, and desire to learn
• Ability to exercise independent judgment and initiative
• Commitment to participate in education of learners (students and new employees)
