Evening (3:00pm-11:00pm) and Night (11:00pm to 7:00am) shifts, Every other weekend/holiday

As an LPN, you will provide professional patient care to long‑term and post‑acute care residents. You will direct and monitor the care provided by CNAs and ensure proper documentation and recording. Your leadership is crucial for both CNA success and patient care.

• Timely, accurate medication administration of assigned team with concurrent required documentation.
• Perform treatments and document progress per physician orders and facility protocol.
• Ensure concise, pertinent, and complete resident care documentation using computerized medical record process. Initiate appropriate care pathway tools.
• Direct CNA assigned to the team in timely completion of quality individualized personal care in compliance with resident's plans. Ensure CNA documentation is accurate and complete.
• Receive and give relevant written and verbal reports per departmental protocols.
• Contact physicians, transcribe orders, and follow up to institute orders in timely manner.
• Monitor residents and report relevant condition status to charge nurse/supervisor and/or physicians promptly.
• Complete pre‑reviews of medication administration records per facility protocols.
• Complete required MDS documentation and related observations in a timely manner.
• Support the CNA team by assisting in direct ADL care of residents.

• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in State of New York.
• Basic Life Support (BLS) Certification from the American Heart Association (AHA).

• 1 year experience.

M - Medium Work:
Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Requires frequent walking, standing or squatting.
