  Long Term Care Facility (LTC)
• Assignment Duration: 17 weeks (September 29, 2025 – January 24, 2026)
• Weekly
  
  Hours:
  
  40 hours per week
• Shifts:
  
  8-hour shifts;
  Days (7am–3pm) and Evenings (3pm–11pm); includes weekends and holidays as needed
• Estimated Weekly Salary: $1250–$1280

• Active Licensed Practical Nurse (LPN) license in New York State
• BLS certification (American Heart Association required)
• Minimum 2 years of recent experience in long term care, skilled nursing, or geriatric nursing
• Strong clinical skills, critical thinking, and ability to work independently
• Dedication to providing patient-centered care with dignity and respect

• Deliver direct patient care to geriatric residents, including medication administration and daily nursing tasks
• Supervise and collaborate with Certified Nursing Assistants (CNAs) and support Registered Nurses (RNs)
• Manage care for 8–10 residents per LPN, ensuring high-quality clinical support
• Provide wound care and monitor resident health (no trach or acute care required)
• Maintain accurate clinical documentation and comply with facility and state healthcare standards
• Promote a positive, respectful, and safe environment for residents and staff
