Work Hours40 hours per week, Monday–Friday, Day/Evening (2pm–10pm or 10am–10pm)
SummaryAs a Community Health Nurse, you will provide nursing interventions, patient education, and care management to a variety of clients in their homes, promoting good health and individualized care.
Responsibilities- Provide comprehensive assessments of the bio‑psycho‑social needs of acutely ill clients in their homes, perform skilled nursing care and prescribed treatments per MD orders, and deliver education to clients and families.
- Develop and document individualized care plans customized for each patient’s unique needs, with interdisciplinary support and maintain effective communication of patient status and progress.
- Maintain accurate and complete documentation of client care and implementation of physician orders per D.O.H. and program standards.
- Communicate effectively with office and supervisory staff, coordinating client services and referrals efficiently.
- Adhere to required department and system protocols, regulations, and education requirements.
- Diploma or Associate’s Degree in Nursing
- Registered Nurse license in New York State
- Valid NYS Driver’s License
- Bachelor’s Degree in Nursing
- 1 year of nursing experience
- Prior home health, clinical, and direct patient care experience
- Mileage allowance and paid drive time between patient visits
