Coordinate transfer of patients between Strong Memorial Hospital and UR Medicine affiliate hospitals.;
Obtain patient consents for transfers and complete required transfer paperwork.;
Interface with physicians, nurses, social workers, and support staff to facilitate communication during transfers.;
Create transfer center intake for service requests to be triaged and managed by area leadership.;
Arrange transportation for transfer and coordinate RN handoff with nursing staff.
