Nurse Practitioner, Healthcare Nursing, Clinical Nurse Specialist
Location: City of Rochester
Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through the system, facilitate interactions with the care team, increase patient and provider satisfaction, maintain a point of contact with providers, and serve as the first point of contact for patients and families.
Responsibilities- Coordinate and oversee care for patients identified as high‑risk based on health risk assessments.
- Support patients through transitions of care by identifying gaps and coordinating follow‑up.
- Serve as the central point of contact for care coordination at the point of service and through targeted outreach.
- Facilitate access to clinical and ancillary services within and outside the health system to support patients’ care plans.
- Lead coordination of clinical care within the Patient‑Centered Medical Home model.
- Work collaboratively under the general direction of the Nurse Manager and Site Medical Director.
- Participate in care planning, care facilitation, and proactive patient management.
- Manage access and utilization, coordinate across the healthcare continuum, and improve outcomes for high‑risk patients.
- Registered Nurse with a current New York State license (required).
- Minimum of three years of professional nursing experience.
- Outpatient primary care experience preferred.
- Experience in community health, care management, transitional care, and/or disease management preferred.
- Motivated, team‑oriented, and able to work collaboratively in a multidisciplinary environment.
