Licensed Practical Nurse will provide home health services under the direction of a Registered Nurse to the Rochester Regional Certified Home Health Agency (CHHA) clients per Medicare, Medicaid and NYS Department of Health (DOH) regulations. Provide superior customer service by modeling the Brand Promise and Core Values.

• Complete accurately and timely all documentation of assigned clients’ care per Federal and State regulatory standards.
• Participate in the CHHA Service Education Program; maintain updated knowledge base of community health practice and community resources.
• Participate in the holiday/evening/weekend rotation.
• Observe and document signs and symptoms related to patient’s health status and changes in their condition and report this to the registered nurse. Obtain the registered nurse’s assessment and signature on all assigned patients.
• Contribute to the identification of basic physical, emotional and cultural needs of the patient through observations and interactions.
• Provide routine nursing services, information, and education to patients during routine care.
• Collaborate with the appropriate registered nurse to revise care plans when the patient’s needs are not being met or have changed.
• Maintain clinical competency.
• Perform other duties as assigned.

• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in State of New York.
• Basic Life Support (BLS) Certification from the American Heart Association (AHA).
• Valid New York State Driver’s License and ability to pass a driver’s license record background check.

• Minimum of 1 year experience.

Medium work:
Exerting 20 to 50 pounds of force occasionally, 10 to 25 pounds frequently, up to 10 pounds continuously to move objects. Requires frequent walking, standing or squatting. Any physical requirements reported will be considered for accommodations.

$28.51 - $34.68 per hour (City: Rochester, Postal Code: 14607). Pay may be modified and may include additional components.
