The Licensed Practical Nurse (LPN) in General Internal Medicine supports patient visits.
Responsibilities include:
- Review of patient health history, medication review, immunization administration, ear lavage, focused nutrition assessments, and blood pressure visits.
- Telehealth duties: collecting pertinent information ahead of provider visits, researching prescription discrepancies, and facilitating prior‑authorization denials.
- Collaboration with providers, desk operations specialists, patient appointment service specialists, interpreters, and medical secretaries.
- Providing nursing care under the direct supervision of the RN or other assigned supervisor, accepting delegation as needed.
- Collecting data through observation and communicating information to assist in patient assessment and care planning.
- Participating as a member of the health‑care team, supporting delegation from the RN/assigned supervisor and provider.
- Delivering care in accordance with the patient care plan, policies, procedures, and principles of relationship‑based care.
- Exhibiting excellent communication skills, mentoring and instructing abilities, and participation on quality‑improvement projects.
Graduate of a school of practical nursing or passed LPN boards within the last two years; otherwise, one year of LPN experience in an applicable setting is required.
Preferred qualifications include:
- One year of LPN experience within the last five years.
- Excellent verbal and written communication skills.
- Experience working in a team environment.
- Computer skills with prior experience with electronic medical record systems preferred.
- Ability to work daytime hours, with flexibility for staffing needs.
- Ability to adapt to unpredictable situations.
- Graduate of an accredited school of practical nursing preferred.
- Current LPN license by applicable state requirements.
- Valid BLS certification for 90 days post‑start date.
- Additional specialty certification/training as required by the work area.
- Hourly rate: $24.07 - $34.95.
- Full‑time schedule:
Monday – Friday; 8‑hour shifts (variable start times). - No weekends.
- Benefits eligible:
