Primary Care Network; UR Primary Care – Batavia - Nurse Practitioner

Nurse Practitioner (Full Time) position at 601 Elmwood Ave, Rochester, New York. This role works Monday to Friday daytime hours and is part of a patient‑centered medical home model within a semi‑rural, community‑based Family Medicine practice.

• Evaluate and manage acute and chronic illnesses.
• Order, interpret, and communicate diagnostic tests and results.
• Perform complete physical examinations and wellness visits.
• Provide hospital/ED follow‑ups and transitional care management.
• Collaborate with network‑based physicians and medical team regarding patient care.
• Facilitate appropriate referrals to specialists or community‑based agencies.
• Perform office‑based procedures when appropriate.
• Provide patient education and counseling to maintain or restore physical and emotional health.
• Support educational needs of learners (students and new employees) through clinical learning experiences, knowledge sharing, and skill training.
• Establish a team‑supported patient panel.

• Minimum of 2 years of active practice as a Nurse Practitioner.
• Master’s degree in Nursing with a Nurse Practitioner track.
• Good standing New York State Registered Nurse license.
• Good standing New York State Nurse Practitioner license.
• Current national certification as a Nurse Practitioner.
• State‑level NP certification with DEA registration applied for prior to hire/start date.
• Strong verbal and written communication skills.
• Interpersonal effectiveness, customer‑relations skills, and computer competence.
• Excellent communication, collaboration, independent judgment, and initiative.
• Desire to learn and participate in continuous education efforts.
