Compensation Range: $98,010.00 - $
Responsibilities- Provide collaborative, interdepartmental care of adult patients in the Palliative Care Unit (4‑1200) and other hospital units, focusing on end‑of‑life and serious illness care.
- Help manage complicated symptoms, facilitate care‑goals conversations, and provide emotional support to patients, families, and colleagues.
- Consult on and admit patients, conduct follow‑up visits, participate in team rounding, document in the EHR, write orders for Unit 4‑1200 patients, and plan discharges.
- Engage in clinical and educational conferences, rounds, student precepting, multidisciplinary trainee teaching, and support of nurses, clinicians, and staff across the hospital.
- Cross‑train and contribute time in the Emergency Department’s embedded palliative care program and the inpatient advanced heart‑failure palliative care program.
- Nurse Practitioner (NP) – Master’s degree as a Nurse Practitioner, active New York State Registered Nurse license in good standing, active New York State Nurse Practitioner licensure in good standing, and current national certification as an Nurse Practitioner.
- Physician Assistant (PA) – completion of an ARC‑PA approved Physician Assistant program, active New York State Physician Assistant licensure in good standing, and current national certification by the NCCPA (National Commission on Certification of Physician Assistants).
- All candidates must be eligible for New York State NP or PA certification and demonstrate clinical competence, superior verbal and written communication skills, interpersonal effectiveness, computer competence, and a desire to learn.
