Compensation Range: $95,846.00 - $

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities

In a 24/7 environment, ensures patient flow, maintains critical access for specialty service, while balancing regional needs. Facilitates timely admission of patients to SMH. Using clinical judgment and experience, interacts collaboratively with physician/provider teams, regional provider teams, at Strong Memorial Hospital, Affiliates, and Outside Hospitals, staff from the Emergency Department, Inpatient and Ambulatory areas, and Bed Management to ensure timely decision-making relative to disposition and care needs of people requiring inpatient placement.

Facilitates timely transfer of patients from acute care hospitals to either Strong Memorial Hospital or Affiliate Hospitals based on assessment of patient needs and services provided by the various hospitals. Acts as a liaison between referring physicians and accepting physicians to facilitate communication regarding the smooth transfer of acute care patients. Triages appropriately to attending physician and service as required.

Uses clinical judgment to ascertain level of care discrepancies, appropriately seeks direction from internal physician when balancing higher level of care is needs across the Affiliate Hospitals.

Essential Functions
  • Utilizes clinical skills to manage patient flow in an expert manner, following policies and procedures.
  • Recognizes trends or subtle changes inpatient /family response to care and responds effectively.
  • Identifies complex patient problems and collaborates in developing a plan of care based on best practice standards and service availability.
  • Determines best patient placement based on expert clinical knowledge, current occupancy, and staffing.
  • Identifies patient safety issues and implements action plans or educational training based on needs.
  • Demonstrates exemplary unit/service standards of documentation.
  • Performs ongoing comprehensive assessment of regional, institution, department, and service components that contribute to patient flow.
  • Provides information to update practice standards and activities based on best evidence.
  • Evaluates the effectiveness of patient placement based on clinical needs and revises as indicated.
  • Functions effectively with clinical staff as an expert Patient Flow Manager.
  • Functions as a clinical resource person.
  • Collaborates with patient care areas and other departments to ensure the needs of patients are safely met and institutional needs are met.
  • Assumes a leadership role in collaborative practice, patient flow improvements, and other regional care activities.
  • Assists clinical departments in implementing patient flow improvements.
  • Supports flow initiatives.
  • Supports Nursing, Specialty Service and General Services in understanding census limitations.
  • Runs effective Patient Flow, Bed Meetings, maintains patient center focus for bed placement.
  • Makes critical decisions in a timely manner.
  • Exhibits knowledge of, and ensures, compliance with Hospital Policy, Federal and State regulatory requirements.
