LocationEdna Tina Wilson Living Center – 700 Island Cottage Road, Rochester, NY 14612
OverviewProvide professional patient care to long‑term and post‑acute care residents. Lead and supervise CNAs, ensure proper documentation, and monitor resident status.
Responsibilities- Timely, accurate medication administration and documentation for assigned team.
- Perform treatments, document progress per physician orders and facility protocol.
- Ensure concise, pertinent, complete resident care documentation using computerized medical record systems.
- Initiate appropriate care pathway tools.
- Direct CNAs to provide individualized personal care per resident care plans.
- Verify and complete CNA documentation.
- Receive and give written and verbal reports to appropriate staff.
- Contact physicians, transcribe orders, and follow up to institute orders in a timely manner.
- Report monitoring of resident conditions to charge nurse/supervisor and/or physicians promptly.
- Pre‑review medication administration records per facility protocols.
- Complete required MDS documentation and related observations.
- Assist CNA team in direct ADL care of residents.
- Graduate from an accredited School of Practical Nursing.
- Current LPN licensure in the State of New York.
- Basic Life Support (BLS) Certification from the American Heart Association (AHA).
- At least 1 year of experience working with a frail or elderly population.
- Medium physical demands: able to exert 20‑50 lbs occasionally, 10‑25 lbs frequently, or >10 lbs constantly to move objects.
- Requires frequent walking, standing, or squatting.

Pay Range: $28.51 – $34.68 per hour
