  • Work Schedule:

    Full time, 40 hours per week, Monday–Friday 8:00–4:30, no weekends or holidays.
Responsibilities
  • Provide individualized assistance, support, and comprehensive coordination of care for patients and their families/caregivers to eliminate barriers to timely care, facilitate flow through the system, and increase patient and provider satisfaction. Serve as the first point of contact for patients and families.
  • Coordinate medical navigation for patients throughout the pulmonary diagnostic and treatment continuum, supporting them from initial pulmonary referral and lung cancer screening through diagnostic bronchoscopy, diagnosis disclosure, and transition to treatment or ongoing surveillance.
  • Serve as a central point of contact, ensuring timely, coordinated, patient-centered care while collaborating closely with the Nurse Manager, Assistant Nurse Manager, and pulmonary providers.
  • Identify resources for patient self‑management skills and identify barriers to a successful care management path, developing strategies to overcome them.
  • Participate in the development of evidence‑based protocols, policies, workflows, guidelines, and flow sheets within the medical home model.
  • Keep abreast of organizational developments and practices that may impact operations by engaging in continuing education, professional organizations, seminars, and relevant literature.
Qualifications
  • Registered Nurse, BSN/Master’s preferred.
