LPN – Ambulatory Nurse (Primary Care Network)

Full‑time, Regular schedule, 40 hours per week, UR‑Day shift (United States of America)

$26.75 – $36.11 per hour (within mandatory compensation range)

• In an outpatient setting, remain accountable for the provision of direct care to assigned patients.
• Contribute to unit activities and support improvement of the care delivery system.
• Work as a team member and assist other staff with implementing patient care.
• Assume responsibility of care for assigned patients under the direction of the co‑assigned RN or MD.
• Participate in the plan of care, implement appropriate nursing interventions, and contribute to patient/family education through data collection, interdisciplinary team collaboration, documentation, and communication.
• Provide self‑directed patient care, administer medications and vaccines, perform minor laboratory testing, and communicate with patients and providers via EMR.
• Self‑starter, team oriented, and enjoys collaborating with others.
• At least 6 months in direct nursing care preferred but not mandatory.
