Location: City of Rochester
Administer practical nursing care to patients in accordance with physician’s orders under the direction and supervision of a Registered Nurse.
Essential Functions
Under the direct supervision of anRNCase Manager,and within the scope of practice for aLicensed Practical Nurse :
- Provide health education according to the teaching planestablishedby the RN.
- Provideroutine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN.
- Observe signs and symptoms,and report patient’sreactions to treatments including drugs and changes in the patient’s physical or emotional condition,to the physician and RN Case Manager.
- Documentanyservicesrenderedto the patient, changes noted in the patient’s condition,and/or family and home situations.
- Document communication withRN Case Manager and physician.
- Other duties as assigned.

Education Requirements
- Diploma as a Licensed Practical Nurse from an accredited program.
Qualifications and Requirements
- Current, valid LPN license issued from NYS Department of Education.
- Experience in home health care preferred.
- Community health experience preferred.
The LPN is primarily in a non-office setting and may be exposed to outdoor conditions.
The working conditions are classified as medium work:
- Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of forcefrequentlyor constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
