Rochester Regional Health is seeking a registered nurse case manager for the Home Care team in Rochester, NY. This full-time role involves assessing patient and family needs, developing and evaluating a care plan, coordinating interdisciplinary services, and supervising PDNs as needed.
Schedule is Monday-Friday, 8:00am-4:00pm with mileage reimbursement, tuition assistance, transfer opportunities, and shift differentials. Requires NY RN license and valid driver’s license.
