Location: City of Rochester
Rochester Regional Health is seeking a Licensed Practical Nurse for ElderONE in Rochester, NY. This full-time role supports care at home through a PACE model, with day or evening shifts and occasional travel between sites within the Rochester area.
The LPN will perform direct patient care under RN supervision, document observations, and assist in treatment plans. A NY LPN license and BLS are required, with opportunities for education and advancement.
