LPN- Family Medicine

Rochester Regional Health seeks an LPN for our Family Medicine practice in Rochester, NY. In this role, you'll provide direct patient care, administer medications and vaccines, collect vitals, and support exams and minor procedures. You'll document in the EHR, educate patients and families, and help coordinate follow-up care. Responsibilities

• Provide direct patient care for family medicine patients under RN/physician supervision
• Administer medications, vaccines, and treatments per provider orders
• Collect and document vital signs, patient histories, and chief complaints
• Assist providers with exams, procedures, and minor office interventions
• Perform basic clinical tests and accurately record results in the EHREducate patients and families on medications, chronic disease, and follow-up care
• Coordinate appointments, referrals, and follow-up communications
• Maintain exam rooms, supplies, and infection control standards
• Participate in care team huddles and quality improvement initiatives
• Support a safe, inclusive, and patient-centered care environment
