Jackson’s Health:
Family Health NP, PLLC is an independently owned and operated primary care practice located in Brighton, New York. Our practice provides comprehensive primary care to adults age 18 and older and serves a diverse patient population with a wide range of acute and chronic health conditions. We are seeking an enthusiastic, compassionate, and independent Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or Physician Assistant (PA) to join our growing team.

This position is ideal for an experienced provider who enjoys primary care, values autonomy, and is comfortable independently managing a patient panel while collaborating with other members of the healthcare team when needed.

Schedule

Jackson’s Health | Brighton, NY

Full-Time or Part-Time

Salary: $115,000–$135,000 per year

We offer flexibility depending on the needs of the selected candidate:

  • Full-Time: Monday through Thursday, four 10-hour days
  • Alternative Full-Time

    Schedule:

    Monday through Friday, with Friday offering a hybrid/administrative option
  • Part-Time: Approximately 24–30 hours per week
  • Per Diem opportunities may also be considered

Location: Brighton, NY

Specialty: Adult Primary Care

Work Setting: Primarily in-person

Key Responsibilities

The NP/PA will provide comprehensive primary care and independently manage an assigned patient panel.

Responsibilities include:

  • Perform comprehensive histories and physical examinations
  • Diagnose and manage acute and chronic medical conditions
  • Develop evidence-based treatment and follow-up plans
  • Provide preventive care in accordance with current USPSTF and other evidence-based recommendations
  • Order and interpret laboratory testing, X-rays, diagnostic imaging, and other appropriate studies
  • Prescribe medications and other treatments within scope of practice
  • Provide health promotion, patient education, counseling, and risk-reduction interventions
  • Coordinate referrals and consultations with specialists and other healthcare professionals
  • Monitor treatment response and modify care plans as clinically appropriate
  • Complete timely, accurate, and comprehensive clinical documentation
  • Communicate effectively and compassionately with patients, families, pharmacies, specialists, and other members of the healthcare team
  • Assist with insurance documentation and prior authorizations when clinically necessary
  • Respond appropriately to urgent or unexpected changes in a patient's condition
Qualifications

Required:

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Current New York State professional licensure or eligibility for licensure
  • Current national board certification appropriate to profession and specialty
  • Current BLS certification
  • Ability to independently evaluate, diagnose, treat, and manage adult primary care patients
  • Strong clinical judgment, communication, and documentation skills

Preferred:

  • 1+ years of experience practicing as an NP or PA
  • 3 years of NP/PA experience strongly preferred
  • Previous experience in Primary Care, Internal Medicine, Family Medicine, or Urgent Care
  • ACLS certification
  • Experience independently managing a patient panel

Compensation & Benefits

Full-Time Salary: $115,000–$135,000 annually
, based on experience, qualifications, and final schedule.

Compensation for part-time or per diem positions will be adjusted based on the agreed-upon schedule and number of clinical hours.
