Nurse Practitioner - Geriatrics

SUMMARYAs a Nurse Practitioner in Geriatrics, you’ll provide comprehensive primary and acute care to older adults across both Long-Term Care and Assisted Living settings.

This role offers the opportunity to build meaningful relationships with patients while managing a diverse range of acute and chronic conditions. You’ll work alongside a supportive team with strong onboarding and collaboration with nursing, respiratory therapy, facility staff, and experienced geriatric physicians.

Comfortable working collaboratively with interdisciplinary teamsRESPONSIBILITIESPatient Care. Provide primary and acute care for geriatric patients, including assessment and management of acute and chronic conditions, physical examinations, diagnosis, treatment planning, medication management, and follow-up care

Long-Term Care. Provide care 2–3 days per week at Unity Living Center, including management of long-term care residents and patients requiring long-term ventilator support in collaboration with respiratory therapy

Assisted Living. Provide primary care 2–3 days per week to residents in assisted living settings, working closely with facility staff and the Geriatric Medical Director Care Coordination. Collaborate with nursing, respiratory therapy, facility staff, physicians, and other members of the care team to coordinate comprehensive patient care Documentation. Maintain accurate medical records, complete required documentation, and monitor patient response to treatment
