SUMMARYAs a Community Health Nurse, you will focus on providing nursing interventions, patient education, and care management to a variety of clients in our community. This promotes good health and one on one patient care in the comfort of their own home.

At least one year of nursing experienceRESPONSIBILITIESPatient Care & Service. Provide comprehensive assessments of the bio-psycho-social needs of acutely ill clients in their home; perform skilled nursing care and prescribed treatments to clients based on MD orders; provide client and family education in accordance with client assessment and plan of care Planning. Develop and document individualized care plans customized for each patient’s unique needs, with support from the interdisciplinary health team as needed;

maintain effective communication to convey patient health status, treatment plans and progress

REQUIRED QUALIFICATIONS:

Diploma or Associate’s Degree in Nursing Registered Nurse license in New York State.

Valid NYS Driver’s License.

PREFERRED QUALIFICATIONS:

Bachelor’s Degree in Nursing preferred.
1 year of nursing experienced preferred.

Prior home health, clinical and direct patient care experience preferred.

- Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery.

Emotional and cognitive demands are substantial due to rapid decision‑making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. Valid licensure, physical readiness, and flexibility for extended or on‑call schedules are typically required.
