Patient and Family Education Navigator

The Patient and Family Education Navigator provides individualized assistance and comprehensive coordination of care for patients and their families in the NICU. This role is ideal for those with a strong background in NICU nursing who are passionate about enhancing patient and family experiences.

• Provide individualized assistance and support for patients and families.
• Coordinate care to eliminate barriers to timely care.
• Facilitate interactions with the care team to increase satisfaction.
• Work collaboratively with the NICU interdisciplinary team.
• Coordinate and provide patient and family education throughout the NICU stay.
• Support families with complex discharge needs, including home oxygen and feeding tubes.
• Coordinate teaching schedules and provide direct caregiver education.
• Assist with Nurse Navigator functions, including appointment coordination and referrals.
• Identify barriers to discharge and strengthen family preparedness.
• Enhance the overall patient and family experience.

• BSN required

• Minimum of 3 years of NICU nursing experience required

• Active NYS RN license required
