Rochester Regional Health Home Care is hiring an RN Case Manager to deliver comprehensive nursing care to a caseload of patients. You will assess needs, develop and evaluate a Plan of Care, and coordinate interdisciplinary services with the care team and physician input.
Position offers a generous signing bonus up to $20,000, mileage reimbursement, tuition assistance, and full benefits. Work schedule is typically Mon-Fri with driving between patient visits in Monroe County.
