*** Responsibilities:
** GENERAL SUMMARY With great latitude and the expectation to exercise independent clinical judgement to support patient movement:
The Transfer Center /Admitting Clinical Coordinator Registered Nurse (TC/ACC RN) provides direct support to Patient Flow throughout SMH and the Affiliate Hospitals. The TC/ACC RN is accountable for 24/7 patient flow. The TC/ACC RN participates in the management of patient triage, patient movement, mentoring house staff (RN and MD) and Regional MD’s concerning patient transfers, the development and evaluation of census management strategies, and the monitoring of patient flow outcomes.
The TC/ACC RN is accountable for supporting standards of care and providing consultation and expertise in the placement of specialty patients (Cardiology, Neurology, Transplant, Oncology, etc.) as well as managing placements of General Adult Services and Emergency Department patients. The TC/ACC RN is involved in activities at the service, institutional, and regional level, and contributes to goal achievement through personal action, effective communication and the facilitation of others.
The TC/ACC RN participates in community and professional organizations, demonstrating commitment to the promotion of URMC visibility and future.
Shift rotation of days, evenings and nights required. Night shifts have option of remote work location.
** ESSENTIAL FUNCTIONS
**** Clinical Practice**:
* Utilizes Clinical skills to Manage patient flow in an expert manner, following policies and procedures /recognizes trends or subtle changes inpatient /family response to care and responds effectively.
* Identifies complex patient problems and collaborates in developing a plan of care based on best practice standards and service availability; determines best patient placement based on expert clinical knowledge, current occupancy and staffing.
* Identifies patient safety issues and implements action plans or educational training based on needs.
* Demonstrates exemplary unit/service standards of documentation.
* Performs ongoing comprehensive assessment of regional, institution, department, service components that contribute to patient flow.
* Provides information to update practice standards and activities based on best evidence.
* Evaluates the effectiveness of patient placement based on clinical needs and revises as indicated.
** Clinical Coordinator:
