We are Hiring! Job Posting: PREVENTIVE CASE WORKER
Responsibilities- Provides services to clients and their families primarily through face-to-face interactions.
- Services include but are not limited to counseling, training/education, advocacy, home studies, life skills, and completion of documentation required to support client situations.
- Completes assessments, formulates, implements, and monitors service delivery plans appropriate for the client population supported by the program.
- Coordinate service delivery and complete required evaluations and screenings, including medical, educational, legal, law enforcement, vocational and mental health.
- Facilitate and encourage connection with appropriate cultural and community resources.
- Attend appointments required by various courts. May be required to provide transportation to clients and family members attending appointments.
- Work collaboratively with Monroe County Department of Human Services and other service providers.
- Assist clients with crises such as lack of food, housing, abuse, domestic violence, etc.
- May be required to represent the program at county level client service plan meetings.
- Attend and participate in local community events.
- Maintain knowledge of community resources available to the client population.
- Meet regularly with supervisor to evaluate and implement professional growth in practice and performance.
- Inform supervisor in a timely fashion about significant events related to client issues.
- Participate in required Agency/Department/Program meetings, ongoing staff trainings, and program development.
- Initiate professional development through attendance at outside conferences, in-service training, reading and applying current, relevant literature, etc.
- Complete required record documentation in accordance with Program Policies and Procedures.
- Maintain documented client case records that include all significant client interventions.
- Timely preparation and submission of required reports and statistics.
- Participate in relevant Agency meetings and/or trainings.
- Actively pursue development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.
- Support the Agency’s Mission, Vision, Values and Strategic Plan, including Core Competency initiatives such as Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse.
- Education:
Bachelor’s Degree in Social Work, Child Welfare or similar experience preferred. Equivalent combination of education and experience will be considered. - Credentials:
Valid and clean NYS Driver’s License and reliable transportation. - Experience:
Two or more years of experience in the child welfare or community service fields preferred. - Physical Demands/Work Environment:
