Schedule: Monday-Friday, 40 hours
Salary: $21.00/hr.

General DescriptionUnder general supervision, the Case Manager provides intake, assessment, and ongoing case management services to clients. The position maintains strict confidentiality and adheres to ethical standards in accordance with NASW, HIPAA, and other applicable regulations. The Case Manager promotes a trauma-informed care environment and supports effective, collaborative communication among program staff. This role also provides coverage and assistance to other programs assigned by the Program Manager.
Essential Duties and Responsibilities- Maintains a caseload of clients form the STEHP program
- Conducting intake assessment and services to clients referred to STEHP from the shelters, self-referrals and or partnering agencies.
- Completes required intake paperwork and any relevant screenings.
- Complete all client’s paperwork and goals into the HMIS system upon initial intake
- Makes referrals to housing, DHS, education, and civil legal issues regarding evictions.
- Complete letters of requests/referral forms, applications, and other required documents of eligibility to other agencies and or organization to advocate for resources which the client is initiated to.
- Guide clients to complete applications for financial assistance, finding the best fit among community funders based on the client’s needs, goals, and abilities.
- Completes relevant screenings and assessments such as but not limited to psychosocial, mental health, financial status, and employment status.
Creates, implements, and monitors comprehensive, person centered, trauma informed and solution-focused plans and goals. Works with clients on harm reduction and other related goals.
Assist the client in understanding and meeting expectations of service providers, courts, and employers.
May deliver instructions in various topics that support client’s ability to live independently.
- Provides ongoing evaluation, supervision, and development of program activities to assure quality and effectiveness of services provided.
Implements and maintains procedures related to program operations.
Submits required reports and statistics in a timely manner.
Collaborates effectively with program partners to heighten efficiency of joint efforts across multiple organizations
Communicates with appropriate program partners, agency staff, community, and criminal justice agencies to assure ongoing and effective referrals into programs.
Serves as Case Coordinator for clients for program care. Works with the Nurse, Peer, and other stakeholders to ensure clients’ needs are being met. Coordinates and participates in regular case conferences.
Monitors and ensures achievement of program milestones and client outcomes.
Oversees client recordkeeping in compliance with appropriate regulatory agencies.
- Collaborates with referring agencies, funders and all related internal staff to effectively support individual client needs.
Maintains regular contact to coordinate client benefits and address all other treatment needs.
Reports to service providers about significant individual behavior or events that will require an immediate response.
Acquire concrete resources to meet individual needs.
